01:22Replying to @ShladamStrawberry The Worst Plantar Fasciitis Video Ever - PART 2 As promised, here is the general approach I take for plantar fasciitis based around the best practice guideline I cited in Part 1. Hopefully this doesn’t take the throne for worst plantar fasciitis video ever lol it was really hard to cram everything I wanted to say into 90 seconds… To summarize the approach: we’re trying to keep your life as close to normal as possible while time takes care of the rest. I like this approach of scaling things down only as far as we need to rather than just assuming someone needs to be scaled all the way down to zero (stop running/exercising) for 2 main reasons… 1) the path back to where they want to be won’t feel quite so far & 2) it allows the person to start doing what they love right away. By laying out as many variables as possible, we give ourselves a chance to take as small of steps as necessary to keep progressing at a gradual, tolerable rate & stay away from the boom-bust cycle. I have no issue with manual therapy as long as it’s correctly communicated (not a fix, not a magic bullet). I’ve just never found it to be a necessary part of management & if someone wants it there are plenty of self-administered techniques I can show them. 01:45The Worst Plantar Fasciitis Video of All Time Seriously haha he didn’t get one thing right… But I’m not surprised… Of all the pain & injury problems I help people with, the educational content I see on social media for plantar heel pain might be the lowest quality of all. It’s mostly just 10 second videos promising to instantly fix plantar fasciitis by doing a stretch/rubbing technique that they claim will fix a cause that isn’t an actual cause (foot shape, tight/weak muscle, adhesion/knot/trigger point etc.)… In other words, lots of stepping on water bottles, “shaking hands” with your foot… and how ‘bout that guy who was just lightly punching his foot?? Hahaha all while claiming “instant fix” too… man, f*ck these people. Rubbing is fine. If it feels good, do it. If it doesn’t, it doesn’t really matter… It doesn’t “fix” anything anyway, it’s just a short term symptom modifier. BUT, if that’s the only thing you’re doing for self-management… I’d say you’re missing a lot of much more valuable pieces… And the entirety of the evidence-based treatment guidelines. 00:54Replying to @Amy Harber Fibromyalgia PART 2 The exercises I prescribe for my fibromyalgia patients usually looks quite a bit different compared to what I prescribe my other patients… Sometimes it looks like very low intensity exercise… Other times it’s regressed so much that it doesn’t even look like exercise; moreso just “movement.” This video includes a few examples of both from a fibromyalgia exercise protocol I found in a study from last year. Several of the exercises in that protocol don’t really look like exercise… Which is why I wanted to share it. Sometimes that’s where these people need to start… But it’s really incredible how far they can go from there. Many of these people do not respond to pain in the short-term the same way most people do; they don’t get exercise-induce hypoalgesia (pain relief via movement). Instead for them it just hurts… That’s why the “sensitive but safe” part is so important to know. BUT there are studies showing that over the long term, these people are able to achieve the same level of pain adaptations as anyone else. Let me know in the comments if I should make a Part 3 showing the full exercise protocol from that study along with my own personal exercise ideas. 01:12Chiro scam EXPOSED. If this saves one person from my colleagues, it was worth making. I’ve seen people lose thousands of dollars falling for this & I hate it. How much do I hate it? Enough to cite 15 studies that nobody will ever read in a 90 second social media video. I need a damn bibliography for this post. How much do I hate it? Enough to put all that effort into this video knowing full well I’ll probably need to make a whole new video on the same topic next week because this stuff just won’t go away. That shows how convincing these scare tactics are: every single aspect of this scam has been repeatedly debunked for the last 50 YEARS… But somehow it’s still here! Must be a pretty convincing scare tactic… so I apologize in advance for my victim blaming at the end of the vid. That’s also why I have zero tolerance for this stuff when I see it on social media. 01:09Fibromyalgia Does Not Exist… When you talk to as many fibromyalgia patients as I do… This is the kind of sh*t that really pisses me off. Fibromyalgia is brutal, confusing, isolating… If you’re going to claim it doesn’t exist & make all those people feel invalidated… You better come with a good argument & some pretty convincing evidence. You can’t tell me fibromyalgia doesn’t exist & then start talking about trigger points in the next sentence. Fibromyalgia is controversial because it isn’t well understood, but that’s true about most things in the pain world when it comes to mechanisms. It’s a real thing, it’s not all in their head. I’ve seen how awful it can be… You couldn’t fake it or make it up if you tried. 01:06Abs Made Easy Don’t let social media confuse you… Abs are just like any other muscles; they don’t need special treatment. If you’re goal is hypertrophy development (muscle growth), train your abs with weight & progressively overload through a full range of motion. Get close to failure with 1 or 2 exercises for 3-4 sets of 8-12 reps a couple times a week. That should do it. I like weighted cable crunches & hanging leg lifts. Keep in mind that muscle growth will be harder to see if you have a high body fat %. So a caloric deficit might be part of the equation for some folks, depending on your goals. Planks & other isometric movements are fine, but we can get good at them quickly which means it’s hard to get close to failure without holding them for 2 minutes. They can be progressed by adding load & changing the lever arm (move your elbows above your head), but I think isometrics are boring & I’d rather train through a full or partial range instead. You can probably bias exercises to target certain fibers, but I’m not sure that’s really necessary, so I don’t worry much about “upper & lower abs.” #abs #absworkout #core #coreworkout #coreexercises
01:26Gooch Walks for Back Pain If you’ve followed me for a while, you know I HATE exercise videos for pain problems. They lack prescriptive nuance, usually over-promise results & include the term “bulletproofing” (which I also f*cking hate), & overall don’t seem helpful to me. But people seem to want them… so here is the type of exercise video I might feel comfortable doing moving forward. Instead of being lazy & trying to hack the social media algorithms, I’m going to explain what I’m doing. You heard me mention that this was a work-related complaint. Oftentimes in those cases, I take more of a load management advice sort of approach rather than an exercise-based approach. But sometimes we can make a case for trying to slowly build someone’s tolerance for work-related demands that we know aren’t going away anytime soon… This guy NEEDED to be able to handle heavy, uneven loads on his spine, so we decided to attack that gradually & directly.
